Resumen

En los últimos años ha ocurrido una revolución en el abordaje de la artritis reumatoide, cuya consecuencia es la remisión. La inhibición de la interleucina (IL) 6 forma parte de estos cambios. La IL-6 es una citocina clave en la patogenia de la artritis reumatoide. Se puede bloquear mediante anticuerpos monoclonales frente al receptor o a la citocina. Los fármacos que bloquean el receptor son el tocilizumab, con amplia experiencia en práctica clínica, y el sarilumab. El sarilumab se distingue del tocilizumab, entre otros aspectos, porque se emplea únicamente de modo subcutáneo y cada 2 semanas, pues tiene una vida media y una afinidad por su receptor mayores. Sarilumab ha demostrado tasas de eficacia elevadas en pacientes con artritis reumatoide y fallo a metotrexato y a otros fármacos antirreumáticos modificadores de la enfermedad convencionales, fallo a antifactor de necrosis tumoral y en intolerantes a metotrexato. En pacientes con fallo a metotrexato, la remisión (CDAI ≤ 2,8) en la semana 52 fue del 14% en el grupo de sarilumab, frente al 5% en el de placebo. Además, mejora la mayoría de los desenlaces relevantes para el paciente: la función física, la calidad de vida y la productividad laboral. La seguridad del fármaco es similar a la del otro inhibidor de la IL-6, el tocilizumab. La dosis eficaz es de 200 mg subcutáneos cada 2 semanas, y puede reducirse a 150 mg/cada 2 semanas por seguridad. Puede emplearse en monoterapia o combinado con fármacos antirreumáticos modificadores de la enfermedad convencionales sin diferencias en eficacia, y su administración es fácil gracias a un dispositivo autoinyector, así como también dispone de jeringa precargada y es estable fuera de la nevera hasta 14 días a un máximo de 25 °C. La inhibición de la IL-6 como diana terapéutica en el tratamiento de la artritis reumatoide es ya una realidad.

Abstract

In the last few years there has been a revolution in the treatment of rheumatoid arthritis, leading to sustained remission. One of these advances is IL-6 blockade. IL-6 is key cytokine in the pathogenic process of rheumatoid arthritis. This cytokine can be blocked with monoclonal antibodies targeting its receptor or against IL-6 itself. The antibodies blocking the IL-6 receptor are tocilizumab, a drug with more than seven years of clinical experience in Spain, and sarilumab, which has recently been approved by the EMA. Sarilumab is distinguished from tocilizumab, among other factors, because it is only used subcutaneously, every 14 days, given that it has a longer half-life and greater receptor affinity. Sarilumab showed greater efficacy in the subgroups of patients with rheumatoid arthritis and failure to methotrexate, other synthetic DMARDs and anti-TNF agents, as well in patients intolerant to MTX. In patients with inadequate response to MTX, the rate of CDAI ≤2.8 (remission) at 52 weeks was 14% for sarilumab and 5% for placebo. Sarilumab also improved most of the patient-reported outcomes, physical function, health-related quality of life, and work disability. Its safety is similar that of the other IL-6 inhibitor, tocilizumab. The effective dose is 200 mg every two weeks subcutaneously, and it can be reduced to 150 mg every two weeks if there are laboratory abnormalities. It can be used with or without methotrexate or other conventional DMARDs with similar efficacy and its administration is easy due to the autoinjector device; furthermore, a prefilled syringe is also available. The drug is stable for 14 days out of the fridge (25°C max.). IL-6 inhibition as a therapeutic target in the treatment of rheumatoid arthritis is already a reality.
