Abstract

We describe a young man with persistent hyperCKemia and MRI findings initially suggestive of inflammatory myopathy, in whom genetic testing confirmed Bethlem myopathy. This case illustrates how collagen VI-related myopathies can mimic idiopathic inflammatory myopathies (IIM), underlining the need for integrated clinical, imaging, biopsy, and genetic data to ensure diagnostic accuracy and avoid unnecessary immunosuppression.

Presentamos el caso de un varón joven con hiperCKemia persistente y hallazgos en resonancia magnética inicialmente sugestivos de miopatía inflamatoria, en quien el estudio genético confirmó una miopatía de Bethlem. Este caso ilustra cómo las miopatías relacionadas con el colágeno VI pueden simular una miopatía inflamatoria idiopática (IIM), subrayando la necesidad de integrar datos clínicos, de imagen, histológicos y genéticos para un correcto diagnóstico con el fin de evitar un tratamiento inmunosupresor innecesario.
