The results obtained in the questionnaire WPAI-AR show that the patient group who were employed demonstrated greater capacity in performing BDLA compared with the group of unemployed people. A series of factors may be determined which can contribute to explaining this phenomenon, such as lower age, better control of the disease, better quality of life and type of physical work undertaken depending on educational level and functional capacity.

In the prospective study of González et al.8 in Mexican patients it was demonstrate that a quarter of the subjects with RA who were included presented with at least one episode of occupational disability during a year of follow-up whilst 4% developed permanent occupational disability. Our study systematically assesses absenteeism and presenteeism, which had not previously been described in our population.

On studying the group of employed patients we found a positive correlation between the level of RA activity, quality of life and functional capacity with the indicators of performance at work described in the WPAI-AR. This coincides with that found by Chaparro et al.10 in the Argentinean population. Even when the majority of correlations found between RA activity and quality of life were statistically poor, the functional capacity assessed using HAQ-DI had a strong degree of correlation with work capacity, conferring this test with an excellent predictive value for detecting impairment in undertaking these activities. As pointed out by Nagasawa and Kameda,24 different factors such as functionality and an appropriate adherence to treatment have repercussion on carrying out work activities and BDLA.

The negative correlation found between disease duration and percentage of absenteeism from work in the simple could be explained by adaptation to their symptoms, with them being able to undertake activities at work to a greater extent with the passing of time, in contrast to that previously described.9,10 Furthermore, negative correlation found between BMI and disease duration may be explained by RA's rheumatic cachexia which has been reported in the literature.25

The multivariable analysis carried out in this study refers to several comorbidities including diabetes, neuropathy and generalised lupus erythematosus having negative effects on undertaking BDLA. An association has been reported between the presence of comorbidities in patients with RA and poorer overall outcomes.26 However, according to the correlation analyses undertaken, only fibromyalgia predisposed greater impairment in carrying out BDLA, without having any significant impact on employment capacity. The other comorbidities described have no significant impact on overall performance, in contrast to that described by Dougados et al.27 This may be explained by the fact that RA symptoms in the majority of these diseases are treated comprehensively. The type of work carried out by patients with RA (being mainly light in load) may also be the reason for this finding.

We should highlight that there was low use of biological agents in the study population, which may have been associated with socioeconomic factors. On being assessed individually, specific pharmacotherapy for RA treatment was not related to greater difficulty in carrying out work activities. However, greater analysis of this point is required, including the medication used to treat the other existing comorbidities and the role of the polypharmacy in this context, which was associated with poorer outcomes in these patients.28

After correlation analysis no significant relationship could be established between gender, age, physical work demand or educational level and the degree of impairment in work performance of the individuals assessed. It is important to point out that in this study over 90% of patients were women and a large percentage of them had no paid work. Even though this study focused on employed patients, a large number of the sample analysed were working in activities connected to household hygiene and cleaning. In this respect the study highlighted that socioeconomic factors and the level of disease activity have a great impact on the ability to undertake BDLA.29

One of the limitations of this study is that patients were only recruited from one centre of reference and the cross-sectional design did not enable the establishment of a temporary relationship between the effects of the factors assessed and overall work capacity. As there were so many factors affecting the outcome, weighted analysis of all related variables was complex. The tools used did not allow for any discernment between people undertaking employment activates under their own initiative (self-employed) and those working under a contract remunerated by a third party. This is something that needs to be evaluated in future studies, since the patient's employment profile is of influence here.

One of the study strengths is that it is the first study of its type in Mexican patients, due to the number of tools used for assessment and the number of people included, which was greater than any previous publication.

Absenteeism and presenteeism are factors which are present in this RA patient group, mainly for those whose disease is more active and who have a lower functional capacity or poorer quality of life. The level of functional capacity was the most strongly associated factor in this respect. For the sake of reducing the economic and social impact of absenteeism and presenteeism we would recommend assessing the level of performance at work when assessing response to treatment in these patients.
Conflict of interests
The authors have no conflicts of interest to declare.
" "textoCompletoSecciones" => array:1 [ "secciones" => array:11 [ 0 => array:3 [ "identificador" => "xres1268351" "titulo" => "Abstract" "secciones" => array:4 [ 0 => array:2 [ "identificador" => "abst0005" "titulo" => "Objective" ] 1 => array:2 [ "identificador" => "abst0010" "titulo" => "Materials and methods" ] 2 => array:2 [ "identificador" => "abst0015" "titulo" => "Results" ] 3 => array:2 [ "identificador" => "abst0020" "titulo" => "Conclusion" ] ] ] 1 => array:2 [ "identificador" => "xpalclavsec1173951" "titulo" => "Keywords" ] 2 => array:3 [ "identificador" => "xres1268352" "titulo" => "Resumen" "secciones" => array:4 [ 0 => array:2 [ "identificador" => "abst0025" "titulo" => "Objetivo" ] 1 => array:2 [ "identificador" => "abst0030" "titulo" => "Material y métodos" ] 2 => array:2 [ "identificador" => "abst0035" "titulo" => "Resultados" ] 3 => array:2 [ "identificador" => "abst0040" "titulo" => "Conclusión" ] ] ] 3 => array:2 [ "identificador" => "xpalclavsec1173952" "titulo" => "Palabras clave" ] 4 => array:2 [ "identificador" => "sec0005" "titulo" => "Introduction" ] 5 => array:3 [ "identificador" => "sec0010" "titulo" => "Materials and methods" "secciones" => array:4 [ 0 => array:2 [ "identificador" => "sec0015" "titulo" => "Study design" ] 1 => array:2 [ "identificador" => "sec0020" "titulo" => "Patient selection" ] 2 => array:2 [ "identificador" => "sec0025" "titulo" => "Variables" ] 3 => array:2 [ "identificador" => "sec0030" "titulo" => "Data" ] ] ] 6 => array:2 [ "identificador" => "sec0035" "titulo" => "Results" ] 7 => array:2 [ "identificador" => "sec0040" "titulo" => "Discussion" ] 8 => array:2 [ "identificador" => "sec0045" "titulo" => "Conflict of interests" ] 9 => array:2 [ "identificador" => "xack435304" "titulo" => "Acknowledgements" ] 10 => array:1 [ "titulo" => "References" ] ] ] "pdfFichero" => "main.pdf" "tienePdf" => true "fechaRecibido" => "2017-08-25" "fechaAceptado" => "2017-12-06" "PalabrasClave" => array:2 [ "en" => array:1 [ 0 => array:4 [ "clase" => "keyword" "titulo" => "Keywords" "identificador" => "xpalclavsec1173951" "palabras" => array:4 [ 0 => "Rheumatoid arthritis" 1 => "Productivity at work" 2 => "Absenteeism" 3 => "Presenteeism" ] ] ] "es" => array:1 [ 0 => array:4 [ "clase" => "keyword" "titulo" => "Palabras clave" "identificador" => "xpalclavsec1173952" "palabras" => array:4 [ 0 => "Artritis reumatoide" 1 => "Productividad laboral" 2 => "Ausentismo" 3 => "Presentismo" ] ] ] ] "tieneResumen" => true "resumen" => array:2 [ "en" => array:3 [ "titulo" => "Abstract" "resumen" => "Objective
This study assesses the relationship between the ability to perform productive activities and the clinical characteristics of RA, such as disease activity, quality of life, functional capacity, workload, pharmacotherapy, and comorbidities.
Materials and methods
A cross-sectional, observational and descriptive study was conducted.
